First Visit

We are always excited about meeting new patients during their first visit to our office. Your initial appointment will consist of a thorough examination and a discussion of potential treatment options. This important 30-minute visit will give us insight into your orthodontic needs. We will schedule another appointment to compile diagnostic records. These records include X-rays, photos and a digital scan for study models and are necessary for developing the appropriate treatment plan. This records appointment will last approximately one-half hour.

Please Assist Us

Provide the following information for your first visit:

  • Please fill out the New Patient Form and submit it.
You can also download the PDF, print it, fill it out and bring it to your first visit.
  • Please be current with your general dentist by having a good cleaning and check-up less than six months before your first visit with us.
  • Please check with your insurance company to determine your benefits and lifetime orthodontic maximum. Bring your insurance card to the appointment so we can obtain a copy. By having the insurance information at the first visit, we well be able to give you a more accurate estimate of your costs and possible payment plan scenarios.
  • If your general dentist has given you a panoramic X-ray to give to us, bring that as well.

We will address the following questions that cover the basics of orthodontic treatment.
We encourage you to ask questions as well.

  • Is treatment needed now or should treatment be delayed until appropriate growth, tooth eruption
    or other factors have occurred?
  • What treatment procedures will be used to correct the problem?
  • Do any permanent teeth need to be removed?
  • How long will treatment take?
  • How much will it cost?
  • What are my payment options?
  • What appointments do I need to get started?
